Incident Narrative

An employee was operating a forklift to move a bundle of steel from a truck. The bundle shifted on the forks. The employee then got off the forklift to adjust the bundle and the bundle shifted and began to fall off the forks. The employee attempted to get out of the way, but was struck by the bundle and sustained a broken left leg that required hospitalization.
