Incident Narrative

An employee was pushing a cart with a spindle on top of it when the cart wheel hit a bump and the cart tipped over. The spindle fell on the employee's foot, flattening the employee's steel toe and breaking a foot bone. The employee required surgery and overnight hospitalization.
