Incident Narrative

In a cleanout area at a plant, an employee was cleaning out the tailgate of a dump truck that was tilted up. A piece of asphalt in the dump body of the truck broke free, falling and striking the employee in the right leg, ankle, and foot. The employee suffered five broken toes and a broken ankle, as well as a laceration on the right leg requiring stitches, and was hospitalized.
