Incident Narrative

An employee was using a cart to push a turbine diaphragm out of a temporary sandblasting tent when the wheels encountered sandblasting grit on the floor, which stopped the progress of the cart. When the cart stopped, the weight shifted, causing it to tip over and the diaphragm to fall on the employee s right leg, breaking it.
