Incident Narrative
Four employees were working to remove a housing cover from a motor. With one employee at each corner of the housing, they connected the lifting device to a 15-ton electric hoist. They had to rotate the housing 90 degrees to clear the motor. They then brought the housing back to its original alignment to then bring it down to the ground. The housing fell from the rigging device and struck one employee's left arm, catching it against the base of the unit. His ulna and radius were broken and he was hospitalized, requiring surgery.
