Incident Narrative

An employee was removing the fourth stud on a four-hole bolt pattern to separate two vertical sections of pipe when the employee's finger was caught between the flange of the falling lower section of pipe and an adjacent pipe flange. The employee suffered an amputation of the fourth finger's distal phalanx.
