Incident Narrative
An employee was installing an outrigger on a frac unit. The employee climbed a ladder to install the top pivot pin while a loader was positioning the outrigger. The top pin would not fully slide into place because the bottom pin's eye was not aligned properly. The employee dismounted the ladder and pushed the lower part of the outrigger to align the pins and eyes with the mount. When the employee placed the lower pin into the eyes, the top pin fell through the eyes and struck the employee's right hand, smashing the index and middle fingers.
