Incident Narrative

While attempting to align fiber cement boxes weighing 300-400 lbs. on a forklift, the boxes shifted, catching the employee's left hand between the edges of the top and bottom boxes. The employee's left middle finger was amputated at the first knuckle joint and the left ring finger was crushed.
