Incident Narrative

An employee was installing fascia onto the truss inside of a two-story house from an extension ladder 6 feet above the floor. His upper body was leaning over the wall when the ladder slipped. As he slid down, his stomach struck a piece of all-thread rod that was protruding from the wall resulting in a puncture wound and hospitalization.
