Incident Narrative
An employee was driving a loader during a concrete mixing process. While he was dumping the loader bucket on top of a bin, some frozen pieces would not go through the bin's grate. He climbed a ladder to retrieve the pieces. He then slipped off the bin and fell 10 feet to the ground, injuring his ankle.
