Incident Narrative

An employee was on a forklift platform, using the forklift to move vehicle hoods into a dumpster for recycling. A vehicle hood knocked him off the platform, causing him to fall about 5-6 feet to the ground. He suffered a broken collarbone and a laceration to the back of his head requiring stitches.
