Incident Narrative

An employee was standing on a scaffold to set a stone in place. The stone panel slipped out of the synthetic webbing sling and struck the scaffold platform, causing the planks to separate. The employee fell through the scaffold about 35 feet, breaking his right leg and right ankle.
