Incident Narrative

An employee was trying to throw out construction debris contained in a wheel barrow. The employee went through an unlatched gate on the third floor of a five floor building while attempting to enter a hoist. The hoist was located on the second floor. The employee fell three stories, suffering a dislocated shoulder, possible leg fractures, and a sprained wrist. The employee was hospitalized.
