Incident Narrative
An employee was on a scissor lift, breaking loose a 4-inch blow pipe at the flanges. A forklift was holding up the line and as the employee removed the bolts to the spool piece, the line dropped and smashed his finger between the flanges he had unbolted and the flanges of the pipe below. The forklift raised the pipe and when the employee moved his hand back and he suffered an amputation.
