Incident Narrative

An employee was cutting bluestone with a wire saw. As the employee used a crowbar to get the wire out of the block, the piece of stone that was being cut broke in the middle, fell at an angle, and rotated down. It struck the employee's right foot, causing amputations to the second and third toes.
