Incident Narrative

An employee and a work crew were attempting to lower a 10-by-10-foot sheet of steel into an excavation using a hydraulic excavator. The hydraulics in the boom of the excavator failed and the sheet lowered, contacting the floor of the excavation and coming loose from the sling rigging. The steel sheet fell over and struck the employee, resulting in a fractured pelvis.
