Incident Narrative

An employee was shutting down a riser pipe to a fire sprinkler system in a section of a building. After performing the shutdown, he went through a set of double doors and onto a small walkway that led to the mechanical room of the elevator. The elevator's floor had been removed as a result of a construction project, and the employee fell approximately 80 feet down the shaft. He suffered spinal fractures, leg fractures, and a broken arm.
