On April 26, 2016, at approximately 9:30 PM, an employee was spinning a non-energized rotary press by hand to clean excess oil with a rag. The rag was caught in the spinning press, pulling the employee's hand toward the punches. His right middle fingertip was amputated by the downward punch.
