Incident Narrative

An employee had been working on a 12- to 14-feet-high roof and was descending an extension ladder. The ladder was tied off and extended 3 feet above the roof at the time. The ladder slipped out from underneath the employee when he was at the top, and he fell 12 to 14 feet, suffering a hairline fracture of the pelvis and a broken pinky finger.
