Incident Narrative

An employee was hanging dry wall from a 5-ft high scaffold and was found unconscious on the floor with multiple skull fractures by his co-worker who heard the noise from the other side of the wall and came to his aid. The employer assumes the employee was climbing down when the scaffold tipped over and he fell head first onto the concrete floor.
