On December 5, 2015, an employee operating a forklift collided, head-on, with a second forklift operator. The employee was thrown from the vehicle and was struck by his own forklift. He suffered a concussion and a de-gloving injury on his left foot and ankle. He also fractured his left-foot big toe and was hospitalized.
