Incident Narrative

Employees were attempting to insert a section of U-adjuster shackle into a chain link using a sledge hammer. The injured employee was holding the shackle. As a coworker swung on the hammer, the injured employee's hand slipped and got struck by the hammer. His left index finger was partially amputated.
