Incident Narrative

An employee was working with a group of three other employees to strip a column form. The employee was holding the column form upright so that when it loosened it would not fall, while another employee was striking the outside of the form with a metal burke bar to loosen the concrete. The injured employee's left hand slid down the side of the form and the burke bar struck it, causing a laceration and fracture to the left ring finger.
