Incident Narrative

On 20 February 2015, at approximately 1100hrs, employee was holding a pin that was being hammered by a third-party employee. His right, middle distal phalanx was struck by the hammer and crushed. The employee was taken to Midland Memorial Hospital. The distal phalanx was surgically removed on 23 February 2015 during an outpatient procedure.
