Incident Narrative

Two employees were repairing a hub bearing. The machine was locked/tagged out and secured. The first employee attempted to line up the internal keyway slot by reaching in to feel the alignment. The second employee was manually tightening bolts to the hub adapter and when the external housing turned it amputated the tip of the first employee's right index finger.
