Incident Narrative
An employee was unloading a cylinder supply trailer. The base of the 160-pound gas cylinder was sliding across the trailer floor. The employee tried to stop it and their finger got caught between the base and a cradle stand for the cylinders, resulting in a fingertip amputation.
