Incident Narrative
On February 7, 2019, an employee was operating a hoist controller to remove chain slack during hook storage. The employee's left hand held the hoist chain in place while the hook was secured to an I-beam. The employee attached the hoist hook to the storage bar and tightened the slack so the hook would not swing. While tightening the slack, the employee's left little finger was caught between the hoist chain and the I-beam, resulting in a fingertip amputation.
