Incident Narrative

An employee was trying to change the spacer on a bar cut off saw. He reached back to the controls meaning to hit the "spacer toggle button" to adjust the spacer, but hit the "vice close button" instead which made the vice close onto the employee's left hand resulting in a partial amputation of the index and middle fingers.
