Incident Narrative
An employee was prepping an area for excavation in order to install underground conduits. The employee was picking up the access ladder for relocation; the end of the ladder has a pivot point where a bracket mounts to the building. When the employee set the ladder on the ground, the pivot point was in the down location, setting the ladder end off the ground. The ladder shifted, swinging the pivot end down and the employee's hands were pinched in the ladder, resulting in an amputation of the left middle fingertip.
