Incident Narrative

An employee lifted the hinged cover of a peanut bin/hopper and put his left hand in the bin. A moving auger inside the bin caught his left-hand ring finger and amputated the fingertip. The bin cover was not interlocked, and the production line was not locked/tagged out at the time of the incident.
