Incident Narrative

An employee was inspecting a manhole vault at the intersection of Lyman Street and Centre Street in Newton, Massachusetts. As he transitioned from an extension ladder to the ground level, he fell approximately 11 feet, 7 inches, back into the manhole vault, breaking his right ankle and requiring hospitalization.
