On January 23, 2020, an employee used an extension ladder to access the roof of a new one-story house in order to install the dry-in. After stepping onto the roof, he slipped off the roof before he had the chance to connect his lanyard onto the anchor point. He fell about 12 feet to the ground and was hospitalized with a right heel injury.
