An employee was working at a construction site and was exiting a concrete sub-grade tank via an extension ladder. Their foot became caught on the ladder, causing the employee to fall approximately 12 feet back into the tank, landing feet-first. The employee sustained fractures to the left ankle, right ankle, and the L2 vertebra.
