Incident Narrative
An employee was trying to install a new roll of banding metal to the banding roll. A crane operator was lifting the roll on to the machine. When the employee tried to remove the nylon lifting sling, it became stuck. The crane operator tried to remove the stuck sling with the crane. When the operator pulled up on the sling, it pulled the roll of banding off the spool. The banding fell approximately 16 inches onto the employee's foot.
