Incident Narrative

An employee was operating a press brake machine to form 16-gauge cold rolled sheet metal into an 18" x 12" metal pan with a 1-inch lip on all four sides. The bottom foot of the press brake fell out of the machine and landed on the employee's left leg/foot, crushing the foot and lacerating the calf and shin.
