Incident Narrative
An employee was observing palletized material moving down a conveyor. She noticed the sprocket and chain power transmission mechanism rubbing against the conveyor frame. While showing the maintenance lead the problem, she pointed her finger in the direction of the sprocket and chain and set off the photo eye, which activated the chain. Her right middle fingertip got caught and amputated.
