Incident Narrative
An employee sustained an amputation injury to the tip of the middle finger on his left hand while clearing a jam on a Global Partition Slotter, which makes slots in corrugated cardboard. The employee reached into an area 3x4" wide, 2' long while the equipment was running. The equipment was not guarded at the time, nor was it shut down or locked out.
