"EXCAVATOR HIT CORRECTLY MARKED PLASTIC 6-INCH GAS MAIN WHILE EXCAVATING HILLSIDE. EXCAVATOR HAD NOT CALLED FOR LOCATES AND WAS DIGGING ON ANOTHER EXCAVATOR'S MARKS. EXCAVATOR FAILED TO POT-HOLE MARKED UTILITY, FAILED TO USE NON-MECHANIZED EQUIPMENT WITHIN 18-INCHES OF A MARKED UTILITY, AND FAILED TO CONTACT FACILITY OWNER FOR INSPECTOR."
