Incident Narrative
ON JUNE 26, 2023, A ROADWAY MILLING CONTRACTOR DAMAGED AN 8"" BARE STEEL MAIN THAT HAD BEEN IMPROPERLY LOCATED BY COLUMBIA'S SECOND-PARTY LOCATOR. THE MAINLINE WAS SHUT OFF FOR SAFETY RESULTING IN AN 811-CUSTOMER OUTAGE. THE FOLLOWING DAY, COLUMBIA BEGAN CALCULATING THE COST OF RESTORATION TO THESE 811 CUSTOMERS AND DETERMINED THAT THE COST EXCEEDED THE REPORTABLE CRITERIA ESTABLISHED IN 49 CFR PART 191.3. THEREFORE, COLUMBIA PROMPTLY REPORTED THIS EVENT WHEN THE CRITERIA WAS MET. PLEASE CONSIDER THIS A FINAL REPORT.
