Incident Narrative
A BUTT FUSION JOINT ON A 3 INCH PE PLASTIC MAIN FAILED RESULTING IN A LEAK. THE NATURAL GAS MIGRATED INTO A NEARBY RESIDENTIAL BASEMENT AND IGNITED. THE LEAK WAS LOCATED UNDER A TREE WHICH WAS DETERMINED TO BE PLACING STRESS ON THE PIPE AND WAS A CONTRIBUTING FACTOR TO THE JOINT FAILURE. THE MAIN WAS PERMANENTLY REPAIRED.
