"DUKE ENERGY'S INVESTIGATION REVEALED A COUPLE OF FACTORS CONTRIBUTED TO THE CAUSE OF THE INCIDENT. FIRST, THE CONTRACTOR HIRED BY DUKE ENERGY (USIC LOCATING SERVICES, INC.) TO LOCATE THE FACILITIES DID NOT FOLLOW THE PROCEDURE WHEN DEALING WITH AN UNTONEABLE SERVICE. DUKE ENERGY HAS A PROCEDURE THAT REQUIRES THE LOCATING COMPANY TO TAKE ADDITIONAL STEPS WHEN LOCATING UNTONEABLE SERVICES. SECOND, THE PLUMBER PERFORMING THE WORK USED A TRACK HOE TO EXCAVATE INSIDE THE TOLERANCE ZONE INSTEAD OF HAND DIGGING TO LOCATE THE SERVICE."
