ON JUNE 26, 2015 AT APPROXIMATELY 8:30 PM, A CONTRACT EMPLOYEE WAS ATTEMPTING TO REMOVE A 2"" TEMPORARY INTERNAL PIPE PLUG FROM A SYSTEM THAT WAS PRESSURIZED. DURING THIS PROCESS THE VENTING GAS WAS ACCIDENTLY IGNITED BY AN UNKNOWN SOURCE.
