"INTERNAL CORROSION AT 6 O'CLOCK POSITION ON 16"" TANK FILL LINE. FACILITY DOWN AT TIME OF RELEASE. OPERATOR DISCOVERED DURING ROUTINE FACILITY INSPECTION. ORIGINAL DESIGN OF PIPING ALLOWED WATER/SEDIMENT TO BE TRAPPED IN THE LOW POINT OF THE PIPING. MANIFOLD PIPING WAS REDESIGNED AND REPLACED ON 1/18/2012 TO REMOVE THIS LOCALIZED AREA OF PIPING. RE-SUBMITTED ON 3/26/2013 TO INCLUDE PART E5F PER PHMSA REQUEST."
