WHILE PERFORMING ROUTINE MAINTENANCE WORK AN EMPLOYEE LOOSENED A METER BYPASS VALVE WITH AN END CAP ASSEMBLY ON A METER SET WHILE UNDER PRESSURE WHERE THE VALVE INADVERTENTLY SEPARATED FROM THE PIPE. THE ESCAPING GAS IGNITED RESULTING IN A FIRE AT THE BUILDING WALL.
