Incident Narrative
WHILE EXCAVATING THE PIPELINE TO INVESTIGATE A FEATURE DETECTED BY AN IN-LINE INSPECTION, THE PIPE WAS NOTICED TO BE WET AND WEEPING PRODUCT. IT WAS FOUND THAT THE BURIED PIPE WAS SITTING A ROCK THAT CAUSED A DENT. THE ERW SEAM WAS POSITIONED ON THE BOTTOM OF THE PIPE AND THE DENT WAS LOCATED ON THE SEAM WELD. FATIGUE APPEARED TO CAUSE THE SEAM TO CRACK WHICH RESULTED IN A SMALL LEAK. THE LINE WAS SHUT DOWN AND THE PIPELINE WAS REPAIRED BY INSTALLING A FULLY WELDED TYPE B STEEL SLEEVE.
