Incident Narrative

ON APRIL 13, 2010, 4-GALLONS OF ULTRA LOW SULFUR DIESEL (ULSD) WAS LOCATED IN AN EXCAVATED DITCH FOR A NEW M&J GATE VALVE AT THE PORT ARTHUR, TEXAS TANK FARM MANIFOLD. THE INVESTIGATION DETERMINED A BURIED PLUG OFF THE BODY BLEED ON THE VALVE WAS THE SOURCE OF THE LEAK. DUE TO THE LOCATION OF THE VALVE BEING LOCATED INSIDE THE MANIFOLD ALL EXCAVATION OF THE IMPACTED SOIL WAS CONDUCTED BY HAND. LABOR COSTS FOR THE EXCAVATION AND THE DISPOSAL OF THE SOIL IMPACTED FROM THE 4-GALLONS OF ULSD EXCEED $50,000. THEREFORE, NOTIFICATION WAS MADE TO THE NRC WHEN IT WAS KNOWN THE RELEASE COST WOULD EXCEED $50,000. REPORT WAS SUPPLMENTED TO ACCOUNT FOR QUESTION E5F.
