WESTERN COOPERATIVE ELECTRIC WAS INSTALLING POWER POLES ON A LOCATE THAT WAS 23 DAYS OLD AND HAD NOT BEEN UPDATED AS REQUIRED. IN THAT TIME THE MARKINGS THAT HAD BEEN PLACED HAD BEEN DEGRADED DUE TO CATTLE IN THE PASTURE. THEY ACTUALLY STRUCK THE PIPE MOVED, THEN STRUCK THE PIPE AGAIN MOVED, AND STRUCK THE PIPE A THIRD TIME PUNCTURING IT. SUPPLEMENTAL FILED AS NEEDED TO CORRECT INCORRECT DATA FOR SMYS OF PIPE INVOLVED 4/15/11
