"PRIMARY O-RING FAILED ON 6"" CONTROL VALVE ALLOWING ETHYLENE TO BE RELEASED FROM A VALVE BODY BLEEDER. BLEEDER WAS REPLACED WITH A FITTING AND A GAUGE. APPROXIMATELY 6 GALLONS OF ETHYLENE WAS RELEASED."
