5/12/2011-THE FAILURE OCCURRED AS A CIRCUMFERENTIAL LEAK THAT ORIGINATED AT A CIRCUMFERENTIALLY-ORIENTED STRESS CORROSION CRACK (SCC) ADJACENT TO THE THE TOE OF A GIRTH WELD ON THE UPSTREAM PIPE JOINT.
