Incident Narrative
APPROXIMATELY 161 BBLS OF CRUDE OIL WAS RELEASED FROM THE ROOF DRAIN VALVE AT THE BOTTOM OF THE TANK. UPON CLEANING AND DEGASSING THE TANK, IT WAS DETERMINED THAT THE WELD ON THE TANK FOR THE ROOF DRAIN VALVE FAILED, DUE TO A FULL PENETRATION CRACK, CREATING AN OPENING AND CAUSING THE RELEASE.
